	Blue Cross and Blue Shield of Illinois, Blue Cross and Blue Shield of New Mexico, Blue Cross and Blue Shield of Oklahoma, and Blue Cross and Blue Shield of Texas are divisions of Health Care Service Corporation, a Mutual Legal Reserve Company, an Independent Licensee of the Blue Cross and Blue Shield Association.

	Preferred Financial Group HCSC's life insurance subsidiaries, Fort Dearborn Life Insurance Companyand Colorado Bankers Life Insurance Company operate as Preferred Financial Group. Combined Services, LLC As a wholly-owned subsidiary of HCSC’s Fort Dearborn Life Insurance Company, Combined Services, LLC operates extensive broker distribution networks.

	Dental Network of America, Inc. Located in Oak Brook, Illinois, Dental Network of America, Inc. (DNoA) functions as a third party administrator (TPA) for all HCSC dental programs.

	MEDecision A wholly-owned subsidiary, MEDecision, Inc. provides a collaborative health care information exchange service as well as a platform for case management, disease management and utilization management.

	Hallmark Services Corporation Hallmark Services Corporation, a wholly-owned subsidiary of HCSC, provides the Direct Markets divisions of Blue Cross and Blue Shield of Illinois and Blue Cross and Blue Shield of Texas with administration and claim adjudication services for individual policies.

	Availilty, L.L.C., a Delaware Limited Liability Company, is a partially-owned subsidiary of HCSC. Availity operates a health care clearinghouse and Internet-based e-health information services on behalf of its owners and its submitters.

	HCSC Insurance Services Company (HISC)) HCSC Insurance Services Company (HISC), a wholly-owned subsidiary of HCSC, is responsible for HCSC’s Medicare business.

	Prime Therapeutics, LLC Prime Therapeutics, LLC, a partially-owned subsidiary of HCSC, is a pharmacy benefit management company.

	RealMed Corporation RealMed Corporation, a partially-owned subsidiary of HCSC, manages online transactions and information exchange between health providers, payers and patients.

	Med-Vantage, Inc. Med-Vantage, Inc., a partially-owned subsidiary of HCSC, is a health care informatics company that provides consulting and business intelligence applications for health plans, providers and consumers.
